Insurance Benefits Guide

Self-Pay Clients

Payment will be due prior to each visit. We can charge the card on file or you can come in person to pay via check or cash.

$135 for initial visits (60 min)
$60 for follow up visits (30 min and $30/15 min increments)
$300 for Diabetes Self-Management Classes (60 min initial and 9 hrs of group classes)

Out of Network Reimbursement

Depending on your specific plan, we can provide you with a superbill (itemized receipt for healthcare services) and you may be able to file a claim for reimbursement with your insurance company.
