We will provide the most current, evidence based nutritional and diabetes care to your patients and keep you informed with their progress. How to Refer a Patient:
Fax: Please send us your referral along with the client’s most recent progress note (to include pertinent medication list), pertinent labs, and other supporting documentation of diagnosis to (833) 526-6362. Be sure to include the nutrition related ICD-10 diagnostic code related to the reason for referral. If you are unsure, you can always use the preventative code Z71.3 Dietary Surveillance and Counseling for nutrition. For your convenience you can print our Nutrition Counseling or Diabetes Education referral forms (found below). Once we receive your fax, we will contact the patient to schedule and notify you of the scheduled appointment.
